All student-athletes who have not previously played an NCAA Division III sport at Penn State Altoona (including incoming freshmen, first-year newcomers to a team, and transfer students) must have the following two forms completed and submitted to Penn State Altoona Athletic Training prior to the start of practice and competition:

1. Sports Medicine Packet

This packet contains information such as emergency contacts, insurance information, consent for treatment by ATC, and assumption of risk.

2. Physical Exam Form / Sickle Cell Trait Lab Report

This form can be completed by your family physician or the Penn State Altoona Health and Wellness Center. Appointments can be made at the Health and Wellness Center by calling 814-949-5540. No walk-in appointments! New students can be accommodated for physicals during SOAR (Student Orientation, Advising, and Registration) when they are on campus.

This physical form helps to make the sports medicine team and head coach aware of any pre-existing injuries or medical conditions. The certified athletic trainer may contact you if you recently had any surgery, have a pre-existing medical condition, or were diagnosed with a new medical condition to request additional clearances by your surgeon or medical specialist.

The sickle cell test is required for all NCAA Division III student-athletes. This test is an out-of-pocket expense for the student-athlete. It is not covered by Penn State.

Affidavit of No Insurance (under 18 years old)

Affidavit of No Insurance must be signed and notarized for all students who do not have health insurance. There are a number of local notaries in the Altoona area; for assistance, please ask a member of the training staff.

NCAA Medical Exception Form

Note: This form needs to be completed by student athletes prescribed EpiPens, ADHD medication, and/or asthma inhalers. For a complete list of banned substances please refer to the NCAA banned substance link listed above. 
*The use of an anabolic agent, hormone and metabolic modulator, peptide hormone, growth factors, related substances and mimetics must be approved by the NCAA before the student-athlete is allowed to participate in competition while taking these medications.
