All returning student-athletes to Penn State Altoona must complete and submit the following two forms to Penn State Altoona Athletic Training prior to the start of practice and competition:

1. Sports Medicine Packet

This packet contains information such as emergency contacts, insurance information, consent for treatment by ATC, and assumption of risk.

2. Physical Exam Form

This form can be completed by your family physician or the Penn State Altoona Health and Wellness Center. Returning students with an activated user ID (ex: abc1234) may schedule an appointment at the Health and Wellness Center online at altoona.psu.edu/health-wellness/online-access.

This physical form helps to make the sports medicine team and head coach aware of any pre-existing injuries or medical conditions. The certified athletic trainer may contact you if you recently had any surgery, have a pre-existing medical condition, or were diagnosed with a new medical condition to request additional clearances by your surgeon or medical specialist.

Returning student-athletes who have already taken a sickle cell test and have previously submitted those results to the Athletic Training office do not need to take another sickle cell test. The sickle cell test is a one-time only test.

Affidavit of No Insurance

Affidavit of No Insurance (under 18 years old)

Affidavit of No Insurance must be signed and notorized for all students who do not have health insurance. There are a number of local notaries in the Altoona area; for assistance, please ask a member of the training staff.
