Penn State Altoona Athletic Training & Sports Physicals

Deadlines for 2025-26 Medical Paperwork:

Aug. 4: M/W Golf, M/W Soccer, M/W Tennis, W Volleyball, M/W Cross Country
Aug. 18: Baseball, Softball
Sept. 15: M/W Basketball, M/W Swimming, M/W Wrestling, M Volleyball
Oct. 13: Co-Ed Spirit
 
Starting Fall 2025, all new, transfer, and returning varsity student-athletes will need to join SportsWare, the electric medical records system (EMR) that the Penn State Altoona athletic training staff is utilizing for varsity sports participation clearance. No paper copies and/or faxes of physical packets will be accepted starting Fall 2025. Physical forms and sports medicine forms will not be accessible to student-athletes unless you create and are approved for a SportsWare account by a member of the athletic training staff.

In order to join SportsWare, a student-athlete must have the following information readily available:

  1. Your Penn State email address; email addresses such as Gmail, Hotmail, etc. will not be accepted. 
  2. Your Penn State 9-digit ID
  3. Student-athletes must FIRST request to join SportsWare and then be approved by a member of the athletic training staff. After being approved, they can create their passwords, profiles, and assess physical forms.
  4. The school ID is "Penn State Altoona"
  5. After a student-athlete is approved by a member of the athletic training staff, a link will be sent to their Penn State email address. That link is only good for 24 hours after approval.

PLEASE NOTE:

  • It is the student-athlete’s responsibility to have all required forms, including sickle cell test results, turned in to the Athletic Trainer prior to practicing and competition. NO student-athlete will be allowed to participate without clearance from the certified athletic trainer.
  • If you have any questions regarding preliminary requirements, participation, or sports medicine, please contact your team's head coach or certified athletic trainer.
  • Recent surgery or physical therapy must be specified to the athletic trainer. If you have had surgery within the last year, or if you are currently in physical therapy, you will need additional paperwork from your surgeon or prescribing medical provider stating that you are fully cleared to participate or if you have any restrictions. This paperwork is in addition to the physical paperwork packets.
  • Any student athlete taking medication for medical conditions such as ADHD, Asthma, or an EPI-PEN due to a severe allergy must provide additional paperwork from the prescribing medical provider. Please fill out and attach supporting medical documents as per requested in the NCAA medical exception document listed above. 
  • Any changes to a student-athlete's medical history (such as a new medical diagnosis, current treatment for chronic medical condition, or add/change in medication) MUST be updated to their physical documentation. Please contact the athletic training staff as soon as possible to update your physical paperwork.
  • If you have a chronic medical condition which requires medical care/visits on a regular basis, clearance notes will be needed to continue to participate in athletics to ensure your safety and well-being.

Appointments for physical exams can be made at the Sheetz Health and Wellness Center by contacting the office ahead of time. No walk-in appointments! New students can be accommodated for physicals during SOAR (Student Orientation, Advising, and Registration) when they are on campus.
