Deadlines for 2024-25 Medical Paperwork:

Aug. 16 - Baseball, Softball
Sept. 6 - M/W Basketball, M/W Swim, M/W Wrestling, W.Bowling
Sept. 27 - M. Volleyball
Oct. 14 - Cheerleading
After a student-athlete has submitted their required paperwork and a member of the athletic training staff has reviewed it, student-athletes will be notified via their Penn State email only if their submission was complete or additional information is needed. Please check your Penn State email regularly. Please allow ample time for the athletic training staff to process your submission. Late physicals will be processed as time allows but will not be a priority.

PLEASE NOTE:
- It is the student-athlete’s responsibility to have all required forms, including sickle cell test results, turned in to the Athletic Trainer prior to practicing and competition. NO student-athlete will be allowed to participate without clearance from the certified athletic trainer.
- If you have any questions regarding preliminary requirements, participation, or sports medicine, please contact your team's head coach or certified athletic trainer.
- Recent surgery or physical therapy must be specified to the athletic trainer. If you have had surgery within the last year, or if you are currently in physical therapy, you will need additional paperwork from your surgeon or prescribing medical provider stating that you are fully cleared to participate or if you have any restrictions. This paperwork is in addition to the physical paperwork packets.

Any student athlete taking medication for medical conditions such as ADHD, Asthma, or an EPI-PEN due to a severe allergy must provide additional paperwork from the prescribing medical provider. Please fill out and attach supporting medical documents as per requested in the NCAA medical exception document listed above.
- Any changes to a student-athlete's medical history (such as a new medical diagnosis, current treatment for chronic medical condition, or add/change in medication) MUST be updated to their physical documentation. Please contact the athletic training staff as soon as possible to update your physical paperwork.
- If you have a chronic medical condition which requires medical care/visits on a regular basis, clearance notes will be needed to continue to participate in athletics to ensure your safety and well-being.

Appointments for physical exams can be made at the Sheetz Health and Wellness Center by contacting the office ahead of time. No walk-in appointments! New students can be accommodated for physicals during SOAR (Student Orientation, Advising, and Registration) when they are on campus.
